\u00a7 1 General provisions<\/b><\/h2>
  1. These terms and conditions determine the rules of using the Neuro Medica Naturotherapy Institute website, available at <\/span>neuro-medica.pl<\/span>, hereinafter referred to as: Portal.<\/span><\/li>
  2. The administrator of the portal is the Foundation for Integrative Neuroscience, based in Olsztyn, Poland, at Ko\u015bciuszki Street 84a, NIP: 7394012559, hereinafter referred to as \u201cNeuro Medica\".<\/span><\/li>
  3. The portal allows users to schedule neurophysiological consultations online and access information about the activities of the Neuro Medica institute, as well as natural methods of supporting the nervous system and well-being of the body.<\/span><\/li>
  4. Neuro Medica makes every effort to ensure that users' personal data is protected. It will not be disclosed to third parties, except in situations specified by law and necessary for the provision of services.<\/span><\/li>
  5. The content of this website is not medical advice and should not be treated as such.\u00a0<\/span><\/li>
  6. In order to register on the Portal, the user must have an active email account.<\/span><\/li>
  7. Neuro Medica uses cookies to store information about the Portal on the hard drive of the end device of the person visiting the Website. This mechanism enables the effective use of the Website. It has no harmful effect on the operation of the user's end device or the software installed on it. The user has the option to voluntarily consent or refuse to consent to the use of cookies after entering the Portal.<\/span><\/li><\/ol>

    \u00a7 2 Registration and user account<\/b><\/h2>
    1. Creating an account on the portal is free and voluntary.<\/span><\/li>
    2. The user is obliged to provide true personal data necessary for the proper provision of services by Neuro Medica.<\/span><\/li>
    3. The user is fully responsible for maintaining the confidentiality of their login details (username and password).<\/span><\/li>
    4. When completing the registration form, the user has the opportunity to read the Terms and Conditions and decide whether to accept them. To accept the Terms and Conditions, the user must tick the appropriate box on the registration form.\u00a0<\/span><\/li>
    5. Neuro Medica may suspend or delete a user's account if the user violates the provisions of these Terms and Conditions, the law, or the rules of social coexistence.<\/span><\/li><\/ol>

      \u00a7 3 Services<\/b><\/h2>
      1. Neuro Medica provides free services electronically to users of the Portal. These services are available 24 hours a day, 7 days a week.<\/span><\/li>

        2. Newsletter<\/span><\/li><\/ol><\/li>
        3. The Contact Form service allows you to send a message to Neuro Medica using the form available on the Portal. In order to use this service, the user should fill in the form available on the Portal and send the completed form electronically by selecting the proper option in the form.<\/span><\/li>
        4. The Newsletter service consists of sending the user electronic information about offers and news concerning the activities of the Neuro Medica institute. In order to use the service, the user enters their email address via the registration form available on the Portal. After submitting the completed registration form, the user receives an email confirmation of the contract for the provision of the Newsletter service to the email address provided in the registration form. At this point, the contract for the provision of the Newsletter service by electronic means is concluded. The user may unsubscribe from this service at any time by selecting the appropriate link at the end of the e-mail message received as part of the Newsletter or by sending an appropriate statement to the e-mail address of the Portal owner: <\/span>info@neuro-medica.pl<\/span>.<\/span><\/li>
        5. The Portal Owner is entitled to block access to free services in the event of a user violating the law or the provisions of the Terms and Conditions, or taking actions that threaten the security of the Portal (including hacking). Blocking access to free services for the reasons mentioned above shall last for the period necessary to resolve the issue that led to the blocking of access to free services.<\/span><\/li>
        6. Neuro Medica also provides paid services, which are described in detail in the Consultation Terms and Conditions document, which is attached to these regulations.<\/span><\/li><\/ol>

          \u00a7 4 Final provisions<\/b><\/h2>
          1. The customer is not entitled to resell purchased goods or digital content without prior agreement with Neuro Medica.<\/span><\/li>
          2. Any doubts or issues related to violations of these Terms and Conditions should be reported to the following email address: admin@neuro-medica.pl.<\/span><\/li>
          3. Any disputes arising from the conclusion of a sales contract, service contract, or contract for the supply of digital content and digital services shall first be resolved through mediation. If mediation proves ineffective, the dispute shall be resolved in accordance with the procedures provided for in generally applicable legal acts.<\/span><\/li>
          4. The administrator of personal data is the Foundation for Integrative Neuroscience, based in Olsztyn, Poland at Ko\u015bciuszki Street 84a, NIP: 7394012559. Detailed information on the processing of personal data is available in the Privacy Policy, which can be found on the website neuro-medica.pl.<\/span><\/li>
          5. The Terms and Conditions shall enter into force on June 1, 2025. The Administrator reserves the right to amend the Terms and Conditions at any time for important reasons. Contracts concluded before the amendment to the Terms and Conditions shall be governed by the Terms and Conditions in force at the time of conclusion of the contract.             1. These terms and conditions set out the rules for using the services provided online and in person at the office located at G\u0119bika Street 10B, 10-686 Olsztyn.<\/span><\/span>

              2. Us\u0142ugobiorca \u2013 pe\u0142noletnia osoba fizyczna b\u0119d\u0105ca konsumentem w rozumieniu art. 221 ustawy z dnia 23 kwietnia 1964 r. Kodeks cywilny (Dz. U. 2020 r. poz. 1740 z p\u00f3\u017an. zm.) oraz pe\u0142noletnia osoba fizyczna zawieraj\u0105ca umow\u0119 bezpo\u015brednio zwi\u0105zan\u0105 z jej dzia\u0142alno\u015bci\u0105 gospodarcz\u0105, gdy z tre\u015bci tej umowy wynika, \u017ce nie posiada ona dla tej osoby charakteru zawodowego, wynikaj\u0105cego w szczeg\u00f3lno\u015bci z przedmiotu wykonywanej przez ni\u0105 dzia\u0142alno\u015bci gospodarczej,<\/span><\/li>
              3. Us\u0142uga – konsultacja \u015bwiadczona przez Us\u0142ugodawc\u0119 na rzecz Us\u0142ugobiorcy. W ramach Us\u0142ugi Us\u0142ugodawca analizuje informacje dotycz\u0105ce stylu \u017cycia Us\u0142ugobiorcy i udziela informacji dotycz\u0105cych dzia\u0142a\u0144 umo\u017cliwiaj\u0105cych popraw\u0119 jego kondycji psychofizycznej,<\/span><\/li>
              4. Test – dost\u0119pny na Portalu formularz zawieraj\u0105cy test neurofizjologiczny online, za pomoc\u0105 kt\u00f3rego okre\u015bla si\u0119 szacowane poziomy aktywno\u015bci neuroprzeka\u017anik\u00f3w w uk\u0142adzie nerwowym osoby korzystaj\u0105cej z konsultacji,<\/span><\/li>
              5. Regulamin – niniejszy regulamin.<\/span><\/span>

              6. The Neuro Medica Terms of Service constitute an appendix to the Terms of Use of the neuro-medica.pl portal. The definitions used in the Neuro Medica Terms of Use have the same meaning in these Terms of Service, unless stated otherwise.

                <\/span><\/li><\/ol>

                \u00a7 2 Provision of services<\/b><\/h2>
                1. The Service provider offers the following paid services:<\/span>
                  <\/span>a) Neurophysiological Consultation \u2013 the service is provided online or in person.<\/span><\/li>
                2. Online consultations are conducted via the functions available on the Portal and via email. The service recipient fills out and submits the form available on the Portal's website. Then, before the scheduled online consultation, the Customer receives a link to the meeting, which allows them to join the conversation. Current information on the cost of the service is available on the Neuro Medica Institute website.<\/span><\/li>
                3. Consultations in person take place at the office located at G\u0119bika Street 10B, 10-686 Olsztyn, Poland. Current information on the cost of the service is available on the Neuro Medica Institute website.<\/span><\/li>
                4. To schedule a Neurophysiological Consultation, it is advised to contact the Service Provider to arrange an online consultation appointment.<\/span><\/li>
                5. To participate in an online consultation, it is recommended to use an up-to-date web browser and a device that enables voice connection (microphone), as well as have access to the Internet. The use of a webcam is also recommended.<\/span><\/li>
                6. Cancellation or rescheduling of an appointment is possible no later than 24 hours before the scheduled appointment time (excluding weekends and public holidays). If an online consultation is canceled less than 24 hours before the scheduled appointment time, the service fee is non-refundable. It is not possible to reschedule an appointment less than 24 hours before the consultation.<\/span><\/li>
                7. To use the Service in person, you must arrange an appointment with the Service Provider at their office. To do so, we recommend contacting the Service Provider using the contact form available on the Neuro Medica website.<\/span><\/li>
                8. In order to properly perform the Service, the Customer shall provide accurate information about their lifestyle. The Service Provider shall not be liable for any errors related to the performance of the Service if the information provided is incomplete or untrue.<\/span><\/li>
                9. The Service is intended exclusively for individuals with full legal capacity. By entering into a contract for the provision of the Service, the Service Recipient declares that they are of legal age and assume legal responsibility for this declaration.<\/span><\/li>
                10. The information provided during consultations does not constitute medical advice and should not be treated as such.<\/span><\/li>
                11. The service is intended for healthy individuals who wish to improve their health or nervous system functioning. The service is not intended for individuals with diagnosed medical conditions.<\/span><\/li>
                12. If there are any doubts regarding the possibility of using the Service, the Service Recipient should contact a doctor who will assess their health condition.<\/span><\/li>
                13. The Service Recipient acknowledges that the information provided as part of the Service does not guarantee the results expected by the Service Recipient due to the complexity and multitude of factors that may affect their condition.<\/span><\/li>
                14. The Service Recipient voluntarily provides personal data in the Test, the results of which are analyzed as part of the Service. However, the Service Provider reserves that the provision of relevant personal data by the Service Recipient is necessary for the proper performance of the Service. The administrator of personal data is the Service Provider.<\/span><\/li>
                15. The Customer has the right to withdraw from the contract for the provision of the Service within 14 days from the date of conclusion of the contract. This right does not apply if the Service Provider has fully performed the Service with the explicit consent of the Customer.<\/span><\/li>
                16. The Service recipient may send a statement of withdrawal from the contract to the following e-mail address: admin<\/span>@<\/span>neuro-medica.pl.

                  1. Us\u0142ugobiorca zobowi\u0105zany jest dokona\u0107 op\u0142aty zwi\u0105zanej ze \u015bwiadczeniem Us\u0142ugi.<\/span><\/li>
                  2. W instytucie Neuro Medica akceptowane s\u0105 nast\u0119puj\u0105ce formy p\u0142atno\u015bci za us\u0142ugi:<\/span>
                    1. a) przelew na konto bankowe w walucie PLN<\/span><\/li>
                    2. b) p\u0142atno\u015b\u0107 got\u00f3wk\u0105 podczas um\u00f3wionej wizyty.<\/span><\/li><\/ol><\/li>
                    3. Mo\u017cliwo\u015b\u0107 skorzystania z konkretnej formy p\u0142atno\u015bci jest uzale\u017cniona od wybranej us\u0142ugi. Szczeg\u00f3\u0142owe informacje dotycz\u0105ce p\u0142atno\u015bci dost\u0119pne s\u0105 na Portalu podczas umawiania wizyty b\u0105d\u017a zamawiania Us\u0142ugi.<\/span><\/li><\/ol>

                      \u00a7 4 Przetwarzanie danych osobowych<\/b><\/h2>
                      1. Dane osobowe Us\u0142ugobiorcy s\u0105 przetwarzane i przechowywane w celu w\u0142a\u015bciwego wykonania Us\u0142ugi. Dane te s\u0105 przetwarzane przez czas niezb\u0119dny do wykonania Us\u0142ugi b\u0105d\u017a do czasu rezygnacji z Us\u0142ugi. Po tym czasie dane osobowe Us\u0142ugobiorcy zostaj\u0105 usuni\u0119te b\u0105d\u017a trwale zanonimizowane, czyli poddane procesowi, kt\u00f3ry uniemo\u017cliwia identyfikacj\u0119 Us\u0142ugobiorcy.<\/span><\/li>
                      2. Us\u0142ugobiorca ma prawo do wycofania zgody na przetwarzanie danych osobowych w dowolnym momencie. W tym celu nale\u017cy wys\u0142a\u0107 wiadomo\u015b\u0107 e-mail na adres: admin<\/span>@<\/span>neuro-medica.pl.<\/span><\/li><\/ol>

                        \u00a7 5 Postanowienia ko\u0144cowe<\/b><\/h2>
                        1. Regulamin wchodzi w \u017cycie z dniem 01.06.2025 r.\u00a0<\/span><\/li>
                        2. Us\u0142ugodawca zastrzega sobie prawo do zmiany Regulaminu, w szczeg\u00f3lno\u015bci w przypadku zmiany przepis\u00f3w prawa reguluj\u0105cych kwestie opisane w Regulaminie, a tak\u017ce w razie zmiany warunk\u00f3w \u015bwiadczenia Us\u0142ugi.<\/span><\/li>
                        3. Us\u0142ugodawca poinformuje Klient\u00f3w oraz Us\u0142ugobiorc\u00f3w o zmianie Regulaminu poprzez wys\u0142anie wiadomo\u015bci e-mail. Zmieniony Regulamin wchodzi w \u017cycie w ci\u0105gu 7 dni od momentu opublikowania.<\/span><\/li>
                        4. W przypadku um\u00f3w zawartych przed zmian\u0105 Regulaminu maj\u0105 zastosowanie postanowienia Regulaminu obowi\u0105zuj\u0105ce w momencie zawierania umowy.<\/span><\/li>
